Get StartedUnderstanding Weight Loss Medications
When learning about weight loss solutions, most people’s first inclination is to doubt their efficacy. That’s because these “cure-alls” may not work for everyone. Unlike other weight management options, GLP-1 medications have been specifically developed to help promote weight loss. These medications are prescribed to overweight and obese people with weight-related health conditions who haven’t been able to lose weight and keep it off with diet and exercise alone. GLP-1s contain active ingredients that mimic hormones in the body that regulate the processes that help promote weight loss.
We’ll explore two specific GLP-1 medications and how they work.
Semaglutide
Semaglutide – developed by Novo Nordisk – is marketed under different brand names depending on its approved use. Ozempic® is the version designed for managing type 2 diabetes, helping regulate blood sugar levels while offering the added benefit of weight loss. For patients specifically focused on weight management, semaglutide is available as Wegovy®, which was FDA-approved in 2021 for treating obesity.
Unlike Ozempic, Wegovy is formulated and dosed specifically for individuals who are overweight or obese, whether or not they have diabetes. Wegovy can be prescribed to those who have a BMI of 30 or higher, or a BMI of 27 or higher with weight-related health conditions.
How does semaglutide work?
Semaglutide mimics the action of glucagon-like peptide-1 – a hormone that naturally regulates blood sugar and appetite. It helps stimulate insulin production when food is consumed, allowing the body to better manage blood sugar levels. Additionally, semaglutide slows stomach emptying, which extends the feeling of fullness after meals. By addressing both blood sugar control and appetite regulation, semaglutide has proven to be an effective treatment for individuals with type 2 diabetes and those struggling with obesity or excess weight.
Tirzepatide
Tirzepatide – developed by Eli Lilly – is marketed as Mounjaro® for treating type 2 diabetes and as Zepbound® for chronic weight management. Its approval for both diabetes and obesity treatment underscores its flexibility in addressing metabolic health. To be eligible for Zepbound, patients must either have a body mass index (BMI) of 30 or higher, or a BMI of 27 or above with weight-related health conditions. The distinction between the two brand names reflects the drug's targeted uses: Mounjaro® focuses on blood sugar control for diabetes, while Zepbound® is intended for obesity management.
How does tirzepatide work?
Tirzepatide is a dual-action medication that mimics two hormones: GLP-1 and GIP (glucose-dependent insulinotropic polypeptide). Like semaglutide, it imitates GLP-1 to prompt the pancreas to produce more insulin, signal the brain to recognize fullness, and slow down digestion by delaying gastric emptying. GIP also plays a role by stimulating insulin production in response to meals and further enhancing the feeling of fullness. This combined effect makes tirzepatide highly effective for managing both blood sugar levels and weight.
Is Tirzepatide Better Than Semaglutide, or Vice Versa?
Research comparing tirzepatide and semaglutide shows that both medications are effective for weight loss in adults who are overweight or obese, but tirzepatide tends to deliver better results. In a study that included patients using either tirzepatide or semaglutide – whether or not they had type 2 diabetes – most participants saw at least a 5% reduction in body weight. However, those treated with tirzepatide experienced greater overall weight loss compared to those on semaglutide.
Another study confirmed that individuals on tirzepatide were significantly more likely to achieve meaningful weight loss, with larger reductions in body weight than those on semaglutide. While both drugs provide substantial weight loss benefits, tirzepatide generally shows greater effectiveness, whether used on or off-label for weight management.
What are the Side Effects of Semaglutide and Tirzepatide?
Semaglutide and tirzepatide share similar side effects. Common side effects include nausea, vomiting, diarrhea, constipation, and indigestion – particularly when first starting the medication or adjusting dosages. These symptoms typically lessen as the body adapts to the treatment.
Other potential side effects include abdominal pain, fatigue, and in rare cases, more serious issues such as pancreatitis or gallbladder problems. Since these medications slow gastric emptying, some patients may experience prolonged feelings of fullness, which can lead to reduced food intake or discomfort. It's important to discuss any side effects with your healthcare provider to ensure safe and effective use of these medications.
Cost and Insurance Coverage
The cost of semaglutide and tirzepatide can fluctuate based on various factors, such as which medication you are prescribed and the pharmacy from which you purchase the medication. Generally, branded medications like Wegovy (semaglutide) and Zepbound (tirzepatide) are pricier, while compounded alternatives may provide a more budget-friendly option.
Key Point: What are Compounded Medications?
Compounded medications are custom-made by pharmacists to meet a patient’s unique needs. This process may involve adjusting dosage, converting tablets to liquids for easier swallowing, removing allergens like dyes, combining medications into one, or addressing drug shortages.
During medication shortages, compounded versions of drugs, like semaglutide, can be prescribed when FDA-approved options aren't available. Compounding pharmacies are legally allowed to create alternatives, which has been crucial for patients needing GLP-1 medications. Compounded semaglutide contains the same active ingredient as Ozempic and Wegovy. With tirzepatide no longer in short supply, compounded versions of this medication are generally not permitted, except under limited circumstances. Patients using compounded tirzepatide should consult their healthcare provider about their options.
Additionally, factors like availability, insurance coverage, and pharmacy pricing can affect the total cost. Therefore, it’s essential to discuss your options with your healthcare provider and pharmacy to find the best solution for your needs.
